Thank you for choosing Growing Together Pediatrics as your child's medical home. If you're preparing for your child's first visit, you can do a few things ahead of time. Please provide the following information before or when you arrive for your visit:

• The completed new patient packet (found above)
• Insurance cards
• Complete immunization records
• Medical records from your previous pediatrician
