Filling out the age-appropriate CHADIS questionnaires in our patient portal before an annual well child visit usually takes just 5 minutes. The CHADIS website is fully secure and HIPPA compliant.
GTP has always used screening tools to assist in the early diagnosis of behavioral, developmental, emotional and social concerns but we’ve partnered with CHADIS to give parents and caregivers the flexibility and convenience of quickly completing the questionnaires on any computer, tablet or mobile device.
